We're Presenting Alongside Great Lakes Caring At NAHC

Here's the session description:
Learn about the strategies a large home health provider used in its rehospitalization reduction program. This presentation centers on a five-step process: choosing a technology partner, selecting a process, preparing for push-back, managing and monitoring, and, lastly, evaluating to improve. It will describe the program background, program goals, and roadblocks to success. It will also cover the considerations involved in assessing how the new technology fits into your organization, how you’ll have to change existing processes, and how to gain buy-in from staff. Success, as attendees will see, depends on integrating the technology with people and processes that are already in place.
